A casual, fun, charity golf outing for The Boys & Girls Club of St. Joseph County with Sunny 101.5 legend Jack Reichert at Pipestone Creek Golf Course ( 6768 Naomi Rd, Eau Claire, MI 49111).

Foursome fee is $304, or $76 per person. Make sure you enter your teammates' names below!

Includes: 18 holes with cart, lunch, limited edition logo'd golf items, donation to Boys & Girls Club of St. Joseph County, and opportunities to win prizes!

1st and 2nd place teams will be awarded prizes. Prizes also awarded to closest to pin and longest drive.

In consideration of Sunny 101.5 permitting me/my teammate(s) to participate in the above event, I hereby and for me /my teammate(s) heirs, executors, administrators, assignees and all legal guardians, WAIVE AND RELEASE ANY AND ALL RIGHTS AND CLAIMS OF ANY NATURE, FOUNDED IN WHOLE OR PART UPON ANY NEGLIGENCE, that I/my passenger may have against Sunny 101.5, its' directors, officers, employees, assignees, licensees, volunteers and any cooperating entities, their representatives, heirs, executors, administrators, successors, and assigns (the “Released parties) arising out of or resulting from any and all injuries or damages of any nature, including death, which I/my teammate(s) may suffer while taking part in this event or any activities connected with the event.
